New Client Questionnaire

If it is your first-time meeting with one of our Immigration Attorneys, you will need to fill out a new client questionnaire. You can access the new client questionnaire form by clicking the link below. Once the form is open, save it to your computer and then complete it as accurately as possible. Once completed, simply click the email button on top of the form to email it to our Charlotte immigration office via email.
